Office Policies
Please arrive 10-15 minutes early to fill out the new patient forms and health questionnaire. Please remember to bring the following:
- Driver’s License or State ID
- Referral if required by your insurer
- Insurance information
- Copies of your operation records, medical records, imaging test results (x-rays, MRI, CT scans, etc.), etc. from your previous doctor visits.
Appointments/Cancellations
In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.
